FIELDWORK IV PLACEMENT

Diagnoses: Variety of conditions with mobility and physical needs
Description: During this 8-week placement, I delivered Occupational Therapy services to adults and seniors who have complex medical needs in a community setting by utilizing consultative models of service delivery. I conducted global OT assessments, and I was actively involved in treatment planning and moving and handling interventions in collaboration with clients and their caregivers. I also conducted assessments, prescriptions and requests for mobility aids funding. For my final project, I created an evidence-based resource for staff with information on moving and handling assessments, transfer equipment and materials, and considerations for determining the types of transfer, and for prescribing the different assistive devices.

Description: During this 10-week placement, I worked in a role of a consultant to Academic Success. In this role, I completed a service evaluation project by conducting an environmental scan and Population Needs Analysis. For this purpose, stakeholders including students and staff were engaged in an iterative approach to contribute to the evaluation process. Based on the analyzed data, I provided the organization with recommendations for how to improve their services to best support students with disabilities in navigating between Academic Success and Accessibility Services.

Diagnoses: Acquired Brain Injury due to motor vehicle accidents, stroke, oncology (tumour), drug overdose
Description: During this 10-week placement, I was responsible for completing physical and cognitive screenings and assessments. In addition, I planned client-centred, occupation-based and task-based (i.e., GRASP) interventions in collaboration with interprofessional team members (i.e., Social Work, Physiotherapists, Speech-Language Pathologists, Dieticians, Rehabilitation Assistants). Lastly, I provided the patients with a discharge plan in collaboration with them, their caregivers, and community healthcare providers.

Description: During this 3-week placement I had the opportunity to conduct a telehealth COPM assessment, and provide my two peer students with feedback on their telehealth assessment. In addition, I collaborated with a group of peers to discuss two real-life client cases with an OT clinician, and to share ideas for assessment and intervention plans. I also practiced using the SOAP format to document the reviewed cases and the telehealth assessments. In the course of this placement, I completed five online Assessment and Intervention courses: Canadian Occupational Performance Measure (COPM), Cognitive Orientation to daily Occupational Performance (CO-OP), Functional Independence Measure (FIM), Solution-Focused Coaching (SFC), and Wheelchair Seating & Mobility. Lastly, I tracked my daily workload to manage my time effectively.

Description: I was responsible for the initial interviews and screenings under the supervision of an OT clinician in a Respirology Unit. I communicated and build rapport with patients in a professional manner and gained experience in documenting SOAP notes.
